<br />PROGRAM GOAL, OBJECTIVE(S) & ACTIVITIES (page 2 of 2) <br /> <br />The followina activities will be completed durina start-up: <br />Step 1. Youthworks and the Juvenile Court will refine referral guidelines for the Unruly <br />Diversion Program. <br />Estimated Completion Date: Completed <br />Step 2. Youthworks, the Juvenile Court and the local schools will develop procedures for <br />establishing the necessary baseline data including citations, adjudications, school <br />suspension, and truancy. <br />Estimated Completion Date: Completed <br />Step 3. Youthworks, the Juvenile Court and the local schools will establish methods for <br />documenting any post-referral citations, adjudications and court ordered <br />placements, or any parental authorized interventions such as psychiatric <br />placements, further truancy, further out-of-school suspensions. <br />Estimated Completion Date: Completed <br />Step 4. Youthworks will develop, with the court and schools, an evaluation tool for gauging <br />whether the services provided were perceived by the youth and the family as <br />beneficial and system outcomes such as out-of-home placements, custody <br />changes and recidivism, attachment to school as measured by attendance. <br />Estimated Completion Date: Completed <br /> <br />The followina steps involvina the court and diversion will continue: <br />Step 5. The Juvenile Court will evaluate referrals and will make referrals following a citation, <br />emergency placement or following an informal adjustment hearing. <br />Estimated Completion Date: Ongoing <br />Step 6. Youthworks will jointly develop with the youth and family a plan to be submitted to <br />the court and to document that the family has accepted services. <br />Estimated Completion Date: Ongoing <br />Step 7. Youthworks will case manage a mix of diversion services provided by the agency <br />and other providers. <br />Estimated Completion Date: Ongoing <br />Step 8. Youthworks will schedule meetings with the juvenile court to review the caseload <br />and to review progress with the youth accepted for diversion. <br />Estimated Completion Date: Quarterly <br />Step 9. Youthworks will work with the court, social services, and law enforcement to <br />determine if there are further arrests or further involvement in the juvenile justice or <br />child welfare system. <br />Estimated Completion Date: Weekly or as needed, Ongoing <br /> <br />The followina steps involvina the schools and truancv or suspension will continue: <br />Step 10. The schools will report truancy to Youthworks, and staff will meet with the school, <br />parents and the youth to develop a plan regarding truancy and then monitor <br />attendance and other conditions (counseling, anger management, etc) of the plan <br />Estimated Completion Date: Ongoing <br />Step 11. The schools will refer youth suspended from school to Youthworks for supervision <br />homework assistance, counseling, community service and special assistance <br />(such a writing letters of apology) <br />Estimated Completion Date: Ongoing <br />
