PART IV(a) ~ PROGRAM GOAL, OBJECTIVE(S) & ACTIVITIES (cont.) <br /> <br />The following activities will be completed during start-up: <br /> <br />Step 1. Youthworks and the Juvenile Court will refine referral guidelines for the Unruly Diversion Program. <br /> Estimated Completion Date: Underway <br />Step 2. Youthworks and the Juvenile Court will develop procedures for establishing the necessary baseline <br /> data including citations, adjudications and the risk of out-of-home placement at the time of referral. <br /> Estimated Completion Date: Within 2 weeks <br />Step 3. Youthworks and the Juvenile Court will establish methods for documenting any post-referral <br /> citations, adjudications and court ordered placements, or any parental authorized interventions such <br /> as psychiatric placements. <br /> Estimated Completion Date: Within 2 weeks <br /> Step 4. Youthworks will develop, with the court, an evaluation tool for gauging whether the services <br /> provided were perceived by the youth and the family as beneficial and system outcomes such as out- <br /> of-home placements, custody changes and recidivism. <br /> Estimated Completion Date: Within 4 weeks <br /> <br />The following steps will continue: <br /> <br />Step 5. The Juvenile Court will evaluate referrals and will make referrals following a citation, emergency <br /> placement or following an informal adjustment hearing. <br /> Estimated Completion Date: Ongoing <br />Step 6. Youthworks will jointly develop with the youth and family a plan to be submitted to the court and to <br /> document that the family has accepted services. <br /> Estimated Completion Date: Ongoing <br />Step 7. Youthworks will case manage a mix of services provided by the agency and other providers. <br /> Estimated Completion Date: Ongoing <br />Step 8. Youthworks will schedule meetings with the juvenile court to review the caseload and to review <br />progress with the youth accepted for diversion. <br /> Estimated Completion Date: Quarterly <br /> Step 9. Youthworks will work with the court, social services, and law enforcement to determine if there are <br /> further arrests or further involvement in the juvenile justice or child welfare system. <br /> Estimated Completion Date: Weekly or as needed, Ongoing <br /> <br /> <br />
