E. Financial/BudQet Information 1. Total amount requested from DMF: $10,000.00 (Please complete the attached project budQet form, Appendix B, for the amount being requested from Dakota Medical Foundation.) The amount requested from DMF represents 11.2 % of the total project budget amount of $89,000. (Please attach a total project budGet.) 2. Please outline a proposed grant payment schedule according to your project's cash flow needs. (Dakota Medical Foundation prefers to schedule payment dates throughout the grant period, i.e. monthly, quarterly, or bi-annually versus annually.) The Parenting Resources Center would prefer to receive grant funds on a quarterly payment schedule. 3. Expected date project or expenditure of funds will begin: July 1, 2004 Expected date project will end or funds will be fully expended: June 30, 2005 4. If Dakota Medical Foundation invests in your project, please describe how DMF's grant award would be expended. A DMF grant award would be expended to support parent education in Region V through parenting classes, enhancing the parenting resources book and video library, fulfilling the resource component of the program by addressing needs as they come to the forefront and providing support to parents through information and referrals.
