STM-BRO-0009(030) - CASS COUNTY <br />RCB CULVERT ON MAPLE RIVER, 6 MI. EAST & 6 MI. SOUTH OF ALICE <br />STRUCTURE - 0.047 MI. <br />Enclosed, for signature, is an Amendment to the Construction and Maintenance Agreement for the above <br />referenced project. <br />Please sign the Amendment as the contractor and return it to me at the NDDOT for further processing. <br />Upon our execution of the amendment, a copy will be mailed back to you for your files. <br />If the County hires a Prime Consultant for the Construction Engineering (CE) for this project and if the <br />CE is paid for with ARRA funds, then the Prime Consultant shall comply with the ARRA reporting <br />requirements. ARRA funded projects are designated with a project number beginning with `STM'. <br />NDDOT, pursuant to Section 1201 and 1512 of ARRA, will be reporting monthly employment <br />information to the Federal Highway Administration (FHWA). The Prime Consultant shall submit a <br />Monthly Employment Report to the NDDOT. <br />The Consultant responsible for the CE shall submit monthly employment reports to NDDOT in one <br />complete submittal for all Prime Consultant employees and Sub-Consultant employees working on the <br />ARRA funded project. Monthly employment reports are due within 10 calendar days after the end of <br />every month. Failure to submit the monthly employment report by the 10"' calendar day will result in a <br />contract deduction of $500 per calendar day until the report is submitted to the NDDOT. <br />The County or Consultant Engineer for the local project will review and report any discrepancies on the <br />Prime Contractors monthly employment report to the NDDOT at (ARRAreports@nd.gov). <br />If you have any questions, you may contact me at 328-2217. <br />~Q m.
