<br />PAGE 2 <br /> <br />5. No supplemental billing will be accepted by CCSS without prior notification to <br />Coalition of the need and justification for such a billing and authorization by the <br />Coalition to submit to CCSS. Payment for authorized supplemental billing will be <br />made as part of the next regular claim cycle. <br />6. CCSS will make payments within statutory requirements. <br /> <br />c. CCSS agrees to reimburse the Provider compensation at a rate of $15/hour for up to 10 <br />hours a week for 50 weeks for each of the two years ($15,000 maximum for contract <br />period). As part of the Provider's claim, the following statement must be included on any <br />billing presented to CCSS for payment: <br /> <br />"I certify that the above information is true and correct. I understand that <br />payment of this claim is payment in full. I further understand that any false <br />claims made will constitute a violation that may result in prosecution." <br /> <br />IV. PROVIDER'S UNDERSTANDING OF TERM OF FUNDING: <br />The Provider understands that this agreement is a one-time agreement, and acknowledges that it <br />has been furnished no assurances that this agreement may be extended for periods beyond its <br />termination date. <br /> <br />V. CHANGES: <br />If either party wishes to change this agreement, such change shall be effective only when <br />mutually agreed to and incorporated by written amendment to the agreement. <br /> <br />VI. AUTHORITY TO CONTRACT <br />The Provider shall not have the authority to contract for or on behalf of or incur obligations on <br />behalf of the Coalition beyond the scope of the contract. <br /> <br />VII. TERMINATION: <br />The contract may be terminated by mutual consent of both parties, or by either party upon 30 <br />days notice, in writing, and delivered by certified mail or in person. <br /> <br />The Coalition may terminate this contract effective upon delivery of written notice to the <br />Provider or at such later date as may be established by the Coalition if federal or state <br />regulations or guidelines are modified, changed, or interpreted in such a way that the services <br />are no longer eligible for the funding proposed for payments authorized by this contract. <br /> <br />Any such termination of this contract shall be without prejudice to any obligations or liabilities <br />of either party already accrued prior to such termination. <br /> <br />The Coalition by written notice of default, including breach of contract, to the Provider may <br />terminate the whole or any part of this agreement: <br /> <br />A. If the Provider fails to provide services provided under the terms of this contract within <br />the time specified herein or any extension thereof; or <br />
