<br />V. REPORTS <br /> <br />A. Grant Reports and Performance Monitorino. The Grantee shall submit to the Foundation periodic <br />interim reports and supporting information for the expenditure of the grant funds in accordance <br />with the Grant Application, including without limitation: 1) a financial report comparing the <br />amounts actually expended for each item with the budgeted amount approved by the Foundation <br />in connection with the Grant Application, and 2) a final report upon completion of the grant <br />period. Such reports shall be submitted to the Foundation by Grantee in accordance with the <br />reporting schedule listed below: <br /> <br />Program Reports Due- <br /> <br />October 29, 2002 <br />December 31, 2002 <br />April 29, 2003 <br />December 31, 2003 (Final Report) <br /> <br />To include: <br /> <br />1. Identify and provide current and pertinent parenting information and resources for <br />parents in Region V so they can raise their children to become self-reliant and <br />successful adults. <br />2. Provide prevention-focused parent education and support opportunities to families in <br />Region V. <br />3. Act as a central clearinghouse to all Region V agencies, individuals, and families by <br />continuing the publication of the "Parenting Essentials & Extras" newsletter. <br />4. Maintain and expand the Parenting Resource Library of books and videos for free <br />checkout to all families, agencies, and individuals in Region V. <br />5. Provide better access to parenting education programs and services of the Parent <br />Resources Center for all individuals and families throughout Region V, regardless of <br />race, age, or economic status. <br /> <br />Grantee shall also submit to the Foundation any other grant reports requested by the <br />Foundation, within a reasonable time period, containing the requested information in the format <br />designated by the Foundation. Grantee shall also supply the Foundation with such information <br />as may be necessary or desirable to permit the Foundation to review the use made of this grant <br />and its effect on the public status of Grantee. <br /> <br />B. Authorized Aoent. For purposes of administration of the reporting and compliance provisions of <br />this agreement, the Foundation's authorized agent is J. Patrick Traynor, President. <br /> <br />C. Grantee's Financial Status and Oroanization. Grantee shall supply the Foundation, upon its <br />request, with any information as may be necessary or desirable to permit the Foundation to <br />review Grantee's financial status, organizational structure and management team. Grantee shall <br />notify the Foundation immediately if there is any substantial negative change in Grantee's <br />financial position and/or management team members or organizational structure. <br /> <br />D. Conflicts of Interest Disclosure Form. Grant Partner acknowledges a continuing duty to disclose <br />Grant Partner's potential conflicts of interest with the Foundation so that the Foundation can <br />protect its tax-exempt status by avoiding transactions, arrangements, or uses of the <br />Foundation's funds that might unreasonably benefit the private interests of any individual, or <br />Grant Partner, including benefits to Grant Partner, Grant Partner's directors, trustees, officers, <br />committee members or key employees, or to immediate family members thereof. Thus, Grant <br />Partner's PresidenUExecutive Director or equivalent thereof, and the PresidenUChair or <br />equivalent thereof, of Grant Partner's Board of Directors, shall complete, or arrange for the <br />completion of the attached Foundation's Conflicts of Interest Disclosure Form ("form") and shall <br /> <br />3 <br />
