<br />These project accounts and records shall be retained by the Grant Partner from the Effective <br />Date of this Agreement and for at least four (4) years following the completion of the grant <br />project period. <br /> <br />B. Audit. The Grant Partner's project accounts and records, including but not limited to accounts <br />and records relating to the project, shall be audited in the same manner as all other accounts <br />and records of the Grant Partner are audited. The Grant Partner's project accounts and records <br />(including but not limited to accounts and records relating to the project) may be audited and/or <br />inspected on the Grant Partner's premises or otherwise by individuals or organizations <br />designated and authorized by Dakota Medical Foundation or Dakota Medical Charities at any <br />time during the period specified in Section III(A) above and within four years thereafter. The <br />Grant Partner agrees to make its books and records available to Dakota Medical Foundation or <br />Dakota Medical Charities for audit at reasonable times and the Grant Partner shall provide all <br />necessary assistance in connection with the audit. <br /> <br />v. REPORTS <br /> <br />A. Grant Reports and Performance Monitorino. The Grant Partner shall submit to Dakota Medical <br />Foundation and Dakota Medical Charities periodic interim reports and supporting information for <br />the expenditure of the grant funds in accordance with the Grant Application, including without <br />limitation: 1) a financial report comparing the amounts actually expended for each item with the <br />budgeted amount approved by Dakota Medical Foundation and Dakota Medical Charities in <br />connection with the Grant Application, and 2) a final report upon completion of the grant period. <br />Such reports shall be submitted to Dakota Medical Foundation and Dakota Medical Charities by <br />Grant Partner in accordance with the reporting schedule listed below: <br /> <br />An interim report is due on December 31, 2003. A final report is due on Auaust 1. 2004. <br />The report(s) should include the following: <br /> <br />A program utilization report setting forth: <br />. An itemized budget outlining how the grant funds were expended; <br />. The number of parenting education opportunities during the grant period; <br />. The number of individuals participating in the parenting education programs; <br />. Copies of any newsletters sent during the grant period; <br />. The number of display contacts during the grant period; <br />. The number of people utilizing the Lending Library; <br />. A brief description of programs within the Parenting Resources Center that were <br />utilized during the grant period; <br />. A list of the agencies collaborating on the program; <br />· The number of parents, families, and professionals served by the program <br />during the grant period; and <br />. The project's effectiveness in providing constructive educational programs for <br />parents, families, and professionals in Region V. <br /> <br />Grant Partner shall also submit to Dakota Medical Foundation and Dakota Medical Charities any <br />other grant reports requested by Dakota Medical Foundation or Dakota Medical Charities, within <br />a reasonable time period, containing the requested information in the format designated by <br />Dakota Medical Foundation or Dakota Medical Charities, as applicable. Grant Partner shall also <br />supply Dakota Medical Foundation or Dakota Medical Charities with such information as may be <br />necessary or desirable to permit Dakota Medical Foundation or Dakota Medical Charities, as <br />applicable, to review the use made of this grant and its effect on the public status of Grant <br />Partner. <br /> <br />All such reports shall be retained in the Grant Partner's files for a period of not less than four (4) <br />years after expiration of the grant period. <br /> <br />3 <br />
