<br />CONTRACT #455-05937 <br /> <br />GRANT AGREEMENT <br /> <br />WHEREAS, the State of North Dakota, acting through its North Dakota Department of Human <br />Services, Children and Family Services Division, herein referred to as "Grantor" has determined the <br />services referred to in the paragraph below entitled "Scope of Service" form an appropriate basis for the <br />expenditure of funds allocated to the Grantor; and <br /> <br />WHEREAS, CASS COUNTY, Cass County Extenstion, P.O. Box 2806 Fargo, NO 58108-3106 <br />herein referred to as "Grantee" proposes to provide those services; <br /> <br />NOW, THEREFORE, the Grantor and Grantee enter into the following: <br /> <br />I. TERM OF THE GRANT <br /> <br />The term of this agreement shall be from the 1st day of July 2005 through the 30th day of <br />June 2006. However, this agreement may be terminated with or without cause upon thirty <br />(30) days written notice by either party. <br /> <br />II. SCOPE OF SERVICE <br /> <br />The Grantee agrees to utilize grant funds to provide child abuse prevention activities, as <br />follows: <br /> <br />A) Provide Parent education; <br />B) Coordinate Circle of Parents Parent Support/Self Help group that meets two or more <br />times per month (one group at a minimum); <br />C) Provide meaningful involvement of parents in the development, operation, evaluation & <br />oversight of the funded programs; <br />D) Participate in collaborative community activities specific to Child Abuse Prevention <br />month; <br />E) Promote the Alliance for Children's Justice Prevention campaign; <br />'F) Identify & address community needs for parent education and support; <br />G) Provide parent education outreach activities for special parent and child populations; <br />H) Provide referrals to social services and community supports; <br />I) Participate in the family resource center network and attend annual meeting; and <br />J) Provide midterm and annual report of budget and activities, which contains the <br />following: <br />i) Numbers and description of families served; <br />ii) Data that demonstrates a high level of satisfaction among families who have <br />used the services; and ' <br />iii) Evaluation data, using the data collection system approved by the Grantor. <br /> <br />A more detailed description of the scope of service is included in the Grantee's Family <br />Resource Center Network Grant Application (2005-2006), which is attached by reference <br />and made a part of this agreement. <br /> <br />III. COMPENSATION <br /> <br />The Grantor, upon written request from the Grantee, shall reimburse the Grantee for <br />allowable expenses incurred, as defined by the OMB Circular cited under Section VIII of <br />this agreement, while performing the scope of service. The total amount of this grant shall <br />not exceed $15,000. No funds will be advanced prior to services actually being provided. <br /> <br />-1- <br />
