expenditures must be submitted within 60 days of project completion or the end of the grant year to receive authorization of full payment. The total amount of the award is $23,000.00. If the Grantee uses grant funds to purchase equipment which in the aggregate costs $1,000.00 or more and the Grantee ceases to provide services within three years, the Grantee shall return all equipment to the Region V esec. IV. FISCAL REPORTING REQUIREMENTS The Grantee must submit fiscal reports on the fonn set forth in Attachment A on a quarterly basis to the Region V csee office for review and approval. A full accounting of documented expenditures must be submitted within 60 days of project completion or the end of the program year to receive authorization of full payment. Expenditure deviations of more than 25% from original budget line items as set forth in the original grant application must receive prior approval from the Region V CSCC Steering Committee. A year~end fiscal report will be reviewed by the Finance Committee and must include a comprehensive review of all program expenditures, grant related income and independent audits/financial compilation of the fiscal agent. V. PROGRAM REPORTING REQUIREMENTS The Grantee must submit program reports on the fonn set forth in Attachment B to the Region V CSCC office for review and approval on a quarterly basis. A year-end program evaluation will be reviewed by the Program Development Committee and must include a comprehensive review of the services and programs offered and a review of measurement outcomes as they relate to the overall plan, goal and program objectives. VI. GRANTEE'S UNO ERST ANDING OF TERM OF FUNDING The Grantee understand that this agreement is a one time agreement and acknowledges that it has been furnished no assurance that this agreement may be extended for periods beyond the termination date of this agreement. VII. GRANTEE'S ASSURANCES The Grantee agrees to comply with the applicable assurances set forth in Attachment C attached hereto. VIII. AUTHORlTV TO CONTRACT The Grantee shall not have the authority to contract for or on behalf of or incur obligations on behalf of the Grantor. The Grantee is not an agent, partner or representative of the Grantor and shall not represent itself as such to any party. The Grantee agrees to be solely responsible for the performance of any subcontractor. 2
