<br />APPENDIX A <br />PAGE I OF 2 <br /> <br />AGREEMENT FOR P ARTICIP A TION <br />IN THE NORTH DAKOTA <br />HIGHWAY SAFETY PLAN <br /> <br />PROJECT NO, PHSP4020404 SP 04 Ph 13 <br /> <br />SCOPE OF WORK <br /> <br />Objective <br /> <br />The purpose of this contract is to provide funding to the Cass County Sheriff's Office for the <br />purchase of equipment to be used for the electronic crash reporting system (TraCS) pilot project. <br />Funding for this project is provided through the North Dakota Highway Safety Plan, The primary use <br />of this equipment must be for submitting motor vehicle crash reports and motor vehicle animal crash <br />reports electronically using TraCS. <br /> <br />Contractual Duties <br /> <br />The Cass County Sheriff's Office will be responsible for ordering and installing the equipment. <br />Proper maintenance of equipment purchased with traffic safety funds will be the direct responsibility <br />of the Cass County Sheriff's Office. The equipment must meet the standards approved by the North <br />Dakota Department of Transportation. Any equipment purchases required to be maintained on <br />equipment inventory must be maintained in good working order and available for inspection. It will <br />be the responsibility of the Cass County Sheriff's Office to ensure officers are trained in the correct <br />operation of equipment and procedures for securing quality crash reports. <br /> <br />The Cass County Sheriff's Office will provide the Drivers License and Traffic Safety Division with a <br />document indicating the cost of investigating and documenting crashes for October I, 2003, through <br />September 30, 2004. <br /> <br />The reimbursement voucher, along with a copy of the vendor invoice, is due no later than 60 days <br />after the end of the month in which expenses occurred. If documentation is not submitted to the <br />Drivers License and Traffic Safety Division by November 14, 2004, payment will be denied, <br />Supporting documentation must be retained in accordance with audit guidelines. <br /> <br />An evaluation report must be submitted by November 14, 2004. This report should include any <br />data relating to the effectiveness of the electronic crash reporting process, such as time/cost <br />savings, improved service, officer satisfaction, etc. Failure to provide this report to the Drivers <br />License and Traffic Safety Division may result in sanctions or elimination from future traffic safety <br />funding opportunities, <br /> <br />All persons reimbursed with traffic safety funds are required to wear safety belts and obey all traffic <br />laws while on official business of the project. The Cass County Sheriff's Office must have a <br />mandatory safety belt policy and provide a copy prior to any reimbursement. <br />
