APPENDIX A DAKOTA MEDICAL FOUNDATION OUTCOMES/RESUL TS PLAN GRID TEMPLA TE A. PROJECT GOAL(S): The Parenting Resources Center will provide researched based, pertinent parent education classes to residents of Region Vand continue to support parents through general information via telephone, resource library, displays and the monthly newsletter. B. OVERALL OUTCOMEJRESUL TS MEASUREMENT: 1. How will you determine your project's success in achieving the health improvement goal? Class evaluations are the primary tool for surveying parent needs and the meeting of these needs by the Parenting Resources Center. Library evaluations will be utilized on an ongoing basis. Partnerships, collaborations and networking will continue to bring new information and feedback on an ongoing basis. Changes in the short term will be evaluated on an ongoing basis. A random survey of past class participants (past 2-3 years) will be sent out to seek input asking what information they continue to use. The class evaluation will be utilized for this purpose also. 2. Identify the data collection tools you will use to measure/verify whether health improvement has occurred: Primary evaluation tools for this program will be the Program evaluation tool, sign - in sheets to track class numbers and newsletter interest, lending library evaluations and outcome evaluations and surveys for several programs are available for use dependent on class offerings (dependent on requests). 2.
