CASS COUNTY <br />GOVERNMENT <br />MEMO <br />TO: Cass County Commissioners <br />County FROM: Bonnie Johnson <br />Administrator <br />DATE: March 30, 2009 <br />Bonnie Johnson <br />SUBJECT: Family Medical Leave Act <br />For the first time, in nearly a decade, changes have been made to the Family <br />Medical Leave Act (FMLA) by the federal government. The changes were <br />effective the first of January 2009, however, implementing regulations were <br />received later, and now Cass County has had the opportunity to develop the new <br />forms for requested leave. <br />Three different forms have now been developed as follows: <br />Family and Medical Leave Act (N) <br />Family and Medical Leave Act (E) <br />Family and Medical Leave Act (F) <br />Form "N" means "Notice". This is the routine notice used by every department <br />head to begin the FMLA process. Our existing form in the policy manual will be <br />replaced with the updated notice. Not much has changed, but the form is much <br />longer. The data completed by the employee and department head is mostly the <br />same. The additional length in the form comes from the fact we are now <br />obligated to provide more "items of notice" to the employee making application <br />for Family and Medical Leave. While these provisions were previously contained <br />in the personnel policy manual, they now have to be stated on the form. <br />Form "E" means "Employee". This is the follow-up form to give an employee if <br />the department head needs further information from the employee to determine <br />how to manage a case. <br />Form "F" means "Family".
