<br />Centre <br /> <br />PURCHASE OF SERVICE AGREEMENT <br /> <br />WHEREAS, the County of Cass has determined the services referred to paragraph below <br />entitled "Scope of Service" should be purchased; and <br /> <br />WHEREAS, Centre Inc., a Private non-profit Agency (Provider), proposes to provide these <br />services; <br /> <br />NOW, THEREFORE, the County of Cass and Centre Inc" enter into the following: <br /> <br />AGREEMENT <br /> <br />I. <br /> <br />TERM OF AGREEMENT: <br /> <br />The term of this agreement shall be from the 1 st day of January, 2009 through the <br />31st day of December, 2010. <br /> <br />II, <br /> <br />SCOPE OF SERVICE: <br /> <br />A. <br /> <br />The Provider agrees to provide services set forth. The Provider will accept referral of <br />intoxicated individuals by the Cass County Sheriffs Department to be cared for at the <br />facility located at 123 15th Street North in Fargo. This care is provided by the staff of <br />the Detox Unit and will include safe, monitored detoxification, screening and referral <br />as appropriate. The Provider will accept Cass County inmates for minimum securi~ <br />placement at the facility located at either 123 15th Street North in Fargo or 220 6' <br />Avenue North in Fargo. Provider will provide residential services which may include <br />work-release, day reporting, counseling services, intensive out-patient chemical <br />dependency treatment, chemical dependency aftercare, drug intervention, cognitive <br />re-structuring, drug and alcohol testing, job placement, conflict resolution, and living <br />skills (parenting, money management, & wellness). Additionally, the Provider will <br />accept referrals from the court or the Sheriff' Department who may be supervised as <br />Day Reporters with or without electronic monitoring. Drug and alcohol testing is also <br />conducted on Day Reporting inmates. <br /> <br />III. <br /> <br />COMPENSATION: <br />A. Compensation for the detoxification services outlined under terms of this <br />agreement shall be $13.00 per hour for each referral (not to exceed $65.00 <br />per 24-hour period for each referral). <br /> <br />B. Compensation for the minimum security services outlined under terms of this <br />agreement shall be: <br /> <br />2009: $54.00 per day for each placement. Inmates will pay $20.00 per working day <br />towards this cost. <br /> <br />2010: $56.00 per day for each placement. Inmates will pay $22.00 per working day <br />towards this cost. <br /> <br />This amount is deducted from the monthly billing for each placement. <br />
