<br />NORTH DAKOTA STATE Parent Information Resource Center(PIRC) <br />COLLABORATIVE REIMBURSABLE CONTRACT ADDENDUM 2007-2008 <br />With Cass County Extension Service <br />"Countdown to Kindergarten" <br /> <br />A collaboration reimbursable agreement contract is in place between the Cass County <br />Extension Service, and the North Dakota State PIRC, a project under the Pathfinder Services <br />of North Dakota, Inc. 1600 2nd Ave. SW, Suite 29, Minot, NO 58701; #U310A060021 from <br />Department of Education, Office of Innovation and Improvement. This addendum provides <br />for the addition of an overall project coordinator. <br /> <br />1. SCOPE OF WORK: <br />. The project coordinator will become familiar with and support the facilitators and <br />coordinators at each of the following sites: Ward County ( Sawyer, Surrey); Cass <br />County (Turtle Mountain, Minot Sunnyside Elementary, Nokomis); Dickinson Public <br />Schools Heart River Elementary}; Morton County (Mary Stark Elementary); Williams <br />County Social Services (Williston Wilkinson Elementary and Trenton 8 Mile); for a <br />total of 9 sites. <br />The coordinator will also: <br />. Monitor program delivery and collect data regarding program changes <br />. Review curriculum upon completion of site sessions and make revisions for <br />upcoming year <br />. Guide the overall evaluation and collection of data for analysis at NDSU <br />. Supervise the fulfillment of all contracts with NDPIRC and the delivery of all reports <br />as indicated in individual contracts: <br />Local sites and the North Dakota PIRC agree to the following: <br />~ Language to be used on materials: "Funded by the North Dakota State PIRC in <br />collaboration with NDSU Extension Service" (printed on all handouts, materials, public <br />relations, promotions, etc.) <br />~ Periodic (monthly/quarterly) submission of expenditures and billing. <br />~ Quarterly report reflecting numbers of families and any required reporting for the <br />PIRC performance report. <br />~ Relevant and required evaluation, data, feedback from Ward County Extension for the <br />NO PIRC's yearly performance report. NO PIRC's yearly performance report is due by <br />August 1, 2008. (Department of Education always reserves the right to change date). <br />~ <br />2. ALLOWABLE COST AND PAYMENT: This contract will have the following budget of <br />$8,500: <br />Coordinator fee $8,000 <br />$20 per hour <br />Travel $ 500 <br /> <br />TOTAL $8,500 <br />
