Request for Proposals (RFP) were to be submitted by
November 30,2007, and too companies responded.

The Village and Lutheran Social Services both sent RFP's for consideration.
Based on the responses received and the reference checks with other political
subdivisions, I suggest the board approve the contract with The Village for
services in 2008. The funds for the new service are included in the personnel
budget.

SUGGESTED MOTION:
Move to authorize the chairman to sign a contract with The Village for an
Employee Assistance Program, effective January 1, 2008.

K:\BJ\EAP MEMO 12-7-2007 .doc
