PURCHASE OF SERVICE AGREEMENT WHEREAS, the County of Cass has determined the services referred to paragraph below entitled "Scope of Service" should be purchased; and WHEREAS, Centre Inc., a Private non-profit Agency (Provider), proposes to provide these services; NOW, THEREFORE, the County of Cass and Centre Inc., enter into the following: AGREEMENT II. III. TERM OF AGREEMENT: The term of this agreement shall be from the 1st day of January, 2003 through the 31 st day of December, 2004. SCOPE OF SERVICE: The Provider agrees to provide services set forth in attached proposals. The Provider will accept referral of intoxicated individuals by the Cass County Sheriff's Department to be cared for at the facility located at 123 15th Street North in Fargo. This care is provided by the staff of the Detox Unit and will include safe, monitored detoxification, screening and referral as appropriate. The Provider will accept Cass County inmates for minimum security placement at the facility located at 123 15th Street North in Fargo. The supervision and programs are provided by the staff of the Comprehensive Sanction Center (CSC) and will include all Iodqinq, food, supervision, dru.q and alcohol testinq, iob placement, counselinci and special programminc~ (e.g. chemical dependency treatment, anc~er management treatment, Coqnitive Re-Structuring, Livincl Skills ('parenting, money manaclement & wellness), etc.) as appropriate. Additionally, the Provider will accept referrals from the court or the Sheriff's Department who may be supervised as Day Reporters with or without electronic monitoring. All drug and alcohol testinq, counselinq and other services are also made available to Day Reportincl inmates. COMPENSATION: Compensation for the detoxification services outlined under terms of this agreement shall be $10.00 per hour for each referral (not to exceed $45.00 per 24-hour period for each referral). Compensation for the minimum security services outlined under terms of this agreement shall be $43.00 per day for each placement. Inmates will pay $17.00 per working day towards this cost. This amount is deducted from the monthly billing for each placement.
