<br />17 <br />information about invasive species, seedling survival following planting, seedling density, and <br />diversity of herbaceous and woody species. Subsequent mandatory reports shall include the <br />same information except for seedling survival information and be submitted on or before <br />December 31st for years two, three, six, and ten (total of five reports). Additional reports will be <br />required every five years until the functional value of the forest mitigation can demonstrate that it <br />has replaced that of the forest impacted by the FMM Project. <br /> <br />Monitoring reports shall contain the following information and any additional information <br />necessary to evaluate the performance of the reforestation site: <br /> <br />1. Name of party responsible for conducting the monitoring and the date(s) the <br />inspection was conducted. <br />2. A brief paragraph describing the mitigation acreage and type of resources authorized <br />to compensate for the impacts. <br />3. Written description of the location of the compensatory mitigation project including <br />information to locate the site perimeter(s), and coordinates of the mitigation site <br />(expressed as latitude, longitude, UTMs, state plane coordinate system, etc.) <br />4. Dates the compensatory mitigation project commenced and/or was completed. <br />5. Short statement on whether the performance standards are being met. <br />6. Summary data, including percent cover of invasive species, species diversity of <br />herbaceous and woody species, and seedling densities. <br />7. Mandatory set points for photos to provide visual documentation of changes and <br />growth over time. <br />8. Maps showing the location of the reforestation site relative to other landscape <br />features, habitat types, locations of photographic reference points, transects, <br />sampling data points, and/or other features pertinent to the mitigation plan. <br />9. A summary of the amounts and type of habitat restored, enhanced, and created at <br />the site. <br />10. Dates of any recent corrective or maintenance activities conducted since the <br />previous report submission. <br />11. Specific recommendations for any additional corrective or remedial actions. <br /> <br />7.3 Performance Standards <br />The results of monitoring will be compared to the performance standards to evaluate the <br />effectiveness of the mitigation and determine if additional planting or maintenance measures are <br />necessary. <br /> <br />Performance standards are survivorship and functionally based, whereas each forest strategy <br />must develop into a representative community that is sufficient in density and diversity to <br />provide the intended habitat function. <br /> <br />7.3.1 Bank Stabilization <br />Bank stabilization sites must meet 80% survivorship of woody vegetation at the end of the <br />monitoring period and provide adequate ground coverage to hold stream banks from further
