NOV, 25,2003 9'49AM CASS CTY SOCIAL SVS N0,239 P, 2 PURCHASE OF SERVICE AGREEMENT WHEKEAS, the Cass County Social Service Board (CCSSB) has det~n~ned the s~-zvices referred to in the paragraph below entitled "Scope of Sa-v/ce" should be purchased; and WI-IE~AS, Fargo Cass Public Health ~01 3rd Avenue North Fargo, ND 58102 (Provider), proposes to provide those semric~s; NOW, THEREFORE, thc CCSSB and the Provider enter into thc follow/ng: AGREEMENT L TERM OF TI-rE AGKEBMENT: Thc term of this agreement shall bc from the 1st day of.January 1, 2004 through the Dccemb~ 31, 2004. II. SCOPE OF SERVICE_: The Provider agrees to render in-home s~vices to eligible individuals (Kecipi~-nts) in Cass County, No~h Dakota. The Provider understands and agrees that eligibility will be d~termin~xi solely by CCSSB. Thc Provider agre~ that the specific services to be provided recipients, the definitions of those services, and related manual chapters are those set forth in Addendum B. In rendm'ing services the Provider agrees to abide by the provision~ of the North Dakota Depar~nent of Human Servicm Manual Chapter Policies and Procedures set forth in Addendum C. Th~ Provider understands arid agrees that the CCSSB will inform the provider of the name of the Recipients of scrv/ces under the tc, u'ns of this Agreement. The Provider further agrees to designate an individual to negotiate with a similarly designated individual from the CCSSB for the specific s~rvices to bc pwvided recipients, thc number of units of service to be provided recipients, and the compm~ation for redpicnts as set forth in paragraph III hereim III. COMPENSATION: Compensation trader the terms of this agreement shall not exc~d the maximum amount allowed for the s~,~-vice provided under the terms of this contract. The maximum amount allowed for the service is set forth in Add=ndurn B.
