<br />J'; <br /> <br />42. INVOICING <br /> <br />A. Contractor shall provide to Department an original and two copies of each <br />invoice. <br /> <br />B. Each payment invoice must include the following information: <br /> <br />(1) Invoice date; <br />(2) Name of Contractor; <br />(3) Pounds of Waste collected; <br />(4) Manifest numbers and date of shipment, including bill of lading <br />number and weight of shipment; <br />(5) Waste Profile Numbers as applicable; and <br />(6) Amount due. <br /> <br />43. MERGER CLAUSE <br /> <br />This contract constitutes the entire agreement between the parties. No contract <br />modification binds either party unless in writing, signed by the parties. Such <br />modification, if made, is effective only in the specific instance and for the specific <br />purpose given.
