APPENDIX A

AGREEMENT FOR PARTICIPATION
IN THE NORTH DAKOTA
410 ALCOHOL INCENTIVE PLAN

PROJECT NO. PHSP4100603 SP 01 Ph 07

SCOPE OF WORK (CON'T)

4). An evaluation section. The evaluation will provide an opportunity for improvement
and determining successes. Random surveys will be done to determine public
awareness and opinion. An administrative review should be conducted from compiled
statistics and operational reports. The purpose is to access the effectiveness of the
saturation patrol program.

Because this is a statewide effort, participation by the Cass County Sheriff s Office is critical to the
success of the project. If the contractor is unable to fulfill the commitment to the project, they must
contact the Drivers License and Traffic Safety office as soon as possible.

To successfully deter motorists from driving impaired, a highly visible and well-publicized campaign
must be conducted at the community level and include the "Do Buckle, Don't Booze" message. This
campaign may include news releases; news conferences; TV interview; media ride-along; and radio
announcements. Samples and/or copies of the public information efforts should be included in the
reimbursement voucher and monthly report. These will be included in our final report to NHTSA.

Reimbursement vouchers are due no later than 60 days after the end of the activity in which expenses
occurred. The final voucher is due no later than November 14,2005. Supporting documents must be
retained at the state level in accordance with audit guidelines.

The Cass County Sheriff s Office will provide a copy of their seat belt policy to the Drivers License
and Traffic Safety Division before requesting reimbursement for this activity. All people reimbursed
with traffic safety funds are required to wear safety belts and obey all traffic laws while on official
business of the project.
