PURCHASE OF SERVICEAGREEMENT <br /> <br />CONTRACT # 950-05225 <br /> <br /> WHEREAS, the State of North Dakota, acting through its North Dakota Department of Human <br />Services, Southeast Human Service Center (State), has determined the services referred to in the <br />paragraph below entitled "Scope of Service" should be purchased; and <br /> <br /> WHEREAS, CASS COUNTY, P.O. Box 3106, Fargo, ND 58108-3106, (Vendor) proposes to <br />provide those services; <br /> <br />NOW, THEREFORE, the State and Vendor enter into the following: <br /> <br />AGREEMENT <br /> <br />I. TERM OF THE AGREEMENT <br /> <br />The term of this agreement shall be from the 1st day of July 2003 through the 30th day of <br />June 2005. However, this agreement may be terminated with or without cause by either <br />party giving the other party thirty (30) days prior written notice. <br /> <br />II. SCOPE OF SERVICE <br /> <br />The Vendor agrees to provide Adult Protective Services in Region V according to the <br />following objectives: <br /> <br />OBJECTIVE 1: Provide a program of vulnerable adult services throughout Cass, <br />Ransom, Richland, Sargent, Steele and Traill counties to vulnerable adults who meet <br />program criteria. The program criteria is defined by the Aging Services Division of the <br />North Dakota Department of Human Services which is attached as Attachment "A" and <br />made a part of this agreement. <br /> <br />OBJECTIVE 2: For each client seen, complete the draft vulnerable adult services data <br />collection form developed by the Aging Services Division of the North Dakota Department <br />of Human Services, which is attached by reference and made a part of this agreement. <br /> <br />OBJECTIVE 3: Complete a vulnerable adult services data compilation on a quarterly <br />basis as requested and submit to the Aging Services Division of the North Dakota <br />Department of Human Services. <br /> <br />OBJECTIVE 4: Access services and remedies for clients. This will include staffing <br />vulnerable adult cases on a weekly basis with a representative of the State. <br /> <br />OBJECTIVE 5: Contact the county social service agencies in Ransom, Richland, <br />Sargent, Steele and Traill on at least a quarterly basis to update them on vulnerable adult <br />services in Region V and to ask for their input. <br /> <br />OBJECTIVE 6: Prepare and submit to the Director of the Southeast Human Service <br />Center, a summary of the Vulnerable Adult Program services and findings covering the <br />one-year period from July 1, 2003 through June 30, 2004 and July 1, 2004 through June <br />30, 2005 by the 31st day of July of each year. <br /> <br />All services provided under this agreement shall be carded out according to N.D.C.C. <br />Chapter 50-25.2, which is attached by reference and made a part of this agreement. <br /> <br />-1- <br /> <br /> <br />
