MASTER CONTRACT # 005-01689 ADDENDUM # 455-05161 ADDENDUM TO MASTER GRANT AGREEMENT WHEREAS, on or about July 1, 1997, CASS COUNTY, and the State of North Dakota, acting through its North Dakota Department of Human Services, entered into a Master Grant Agreement, and WHEREAS, now the State of North Dakota, acting through its North Dakota Department of Human Services, Children and Family Services Division, herein referred to as "Grantor" wishes to enter into an addendum to the agreement with Cass County, herein referred to as "Grantee" and WHEREAS, this addendum shall be subject to the terms of the Master Grant Agreement, NOW, THEREFORE, the parties enter into the following additional terms: ADDENDUM 1) TERM OF THE GRANT The term of this addendum shall be from the 1st day of July 2003 through the 30th day of June 2004. However, this addendum may be terminated with or without cause upon thirty (30) days written notice by either party. 2) SCOPE OF SERVICE The Grantee agrees to utilize grant funds for the operation, expansion and enhancement of the community based prevention focused family resource center. The Grantee will: A. Provide support for parents, promote the development of parenting skills, foster the promotion of strong families, and promote family access to formal and informal parenting resources; B. Participate in a network of family resource centers working towards providing/enhancing designated core services; C. Develop and present or participate in community activities specific to child abuse prevention month; D. Promote prevention of child sexual abuse campaign; and E. Work toward the establishment of a parents' mutual self-help group using the "Circle of Parents" model. Reports are due January 30, 2004 and thirty (30) days after the expiration of this addendum. 3) COMPENSATION The Grantor, upon written request from the Grantee, agrees to reimburse the allowable expenses incurred, as defined by the OMB Circular cited under Section VII of the Master Grant Agreement, while performing the scope of service. The total amount of this grant addendum shall not exceed $22,000. No funds will be advanced prior to services actually being provided. Request for reimbursement by the Grantee should be sent monthly to the Grantor. Final reimbursement requests shall be submitted to the Grantor no later than thirty (30) days after the expiration of this addendum. -1-
