Mike Argall <br />Cass County Sheriff's Office <br />Post Office Box 488 <br />Fargo, ND 58107-0488 <br /> <br />TRAFFIC SAFETY CONTRACT FOR FY 2003 - PHSP4020302-02-09 <br /> <br />Enclosed is the Traffic Safety Contract that will provide funding support for the proposed activities. <br />Please review the contract and have it signed by aperson with contracting authority, and witnessed. <br />Please return the signed original, including Appendix A, and required insurance certificates (see <br />next paragraph) to our office. The contract will become effective when the director of the NDDOT <br />signs it. We will return a copy of the fully-executed contract to you, along with a reimbursement <br />voucher. Please make as many copies of the voucher as you may need. <br /> <br />Please pay special attention to the attached "Risk Management Appendix," which is a part of this <br />contract. You are required to provide this office with a copy of the North Dakota Insurance Reserve <br />Fund (NDIRF) or the following, which can be obtained from insurance companies: <br /> <br />Commercial general liability (cannot be canceled or modified without 30 days' prior notice) <br />Automobile liability (cannot be canceled or modified without 30 days' prior notice) <br />Worker's Compensation Insurance Certificate <br /> <br />If subcontractors are hired to do any of the work or supply any materials, we also require these <br />certificates of liability be sent to us. In all cases, your insurance certificates shall name the state <br />as an additional insured and a waiver of subrogation shall be provided. <br /> <br />Periodic desk and on-site monitoring by project managers is required. This contract specifies that any <br />progress reports required will need to be in our office prior to reimbursement. <br /> <br />We are looking forward to the many benefits your program will provide in the traffic safety area.
