Exhibit I <br />Components and Fees for CCSD Inmate Medical Services, Employment Physicals, Clinical <br /> Training, Medical Consultation and On-Call Coverage <br /> <br />This Exhibit shall have an initial term coinciding with the Initial Term as defined by Article I, <br />paragraph 1.1 of the Agreement. <br /> <br />This Exhibit may be amended according to Article VI of the Agreement. <br /> <br />Inmate Medical Services <br />Professional medical services provided by FHC professional staff to CCSD clients will be billed <br />at a rate of $3,855.00 / month, subject to adjustment on July 1 of each year. This fee accounts for <br />professional medical services not to exceed 400 inmate encounters per year. Should volumes <br />significantly increase, FHC would request the opportunity to discuss adjustment if the volumes <br />impact the resources required to deliver the service. The fee includes administrative <br />responsibilities, on-call services (24 hour on-call availability) and clinical review processes as <br />stated in Article I of the Agreement. <br /> <br />Employment Physical (99499.01) <br />Fee = $40.00 / Physical <br /> 1. Record patient's vitals <br /> 2. Record a problem focused history <br /> 3. Conduct an expanded problem focused examination <br /> 4. Review the patient's pertinent past medical, family and social history <br /> 5. Conduct a review of systems with particular focus on cardiovascular and pulmonary health <br /> 6. Examine and question the patient to detect communicable diseases <br /> <br />Medical supplies and diagnostic procedures will be billed separately from professional services <br />and in accordance with the FHC fee schedule. <br /> <br />Clinical Trainim,, Planning. Case Meetings and Chart Review <br />For services rendered outside the scope of this agreement: <br /> 1. The cost for special clinical training, CCSD planning, case meetings or chart review <br /> requests provided by non-physician staff is $35/hour. <br /> 2.
