This <br />contract is between Cass County Jail and Family HealthCare Center. After reviewing <br />proposals for medical services last spring/summer, Family HealthCare Center was chosen <br />due to their familiarity with jail medical services and bringing in the lowest bid for <br />services. <br /> <br />Family Healthcare Center (FHC) has worked with the jail for several years, but the <br />contract has changed fi.om previous years because of requests made by the jail Captain <br />and current nursing staff and changes in FHC's system. The most significant change is <br />that FHC will now be charging a fiat rate for services (including inmate visits, <br />administration time, and on-call services). Please refer to Exhibit I in the contract for the <br />fees. <br /> <br />We will continue to have a medical provider on-site twice weekly. We currently have a <br />Nurse Practitioner here during for the majority of those visits, but we now require the <br />responsible physician be here at least twice monthly so that he may stay more up to date <br />with health services operations. <br /> <br />If you have any questions regarding the contract or services, please contact FHC Director <br />of Operations Shane Waslaski at 239-7171 Ext. 19 or myself at 271-2942 (however I will <br />be out of town until 2-18-03). Once the contact is signed by the commission, Mr. <br />Waslaski requests that a signed copy be returned to him as soon as possible (306 4th St. <br />N.; Fargo, ND 58102).
