DATE INITIATED: 5/14/2015 <br />PROJECT: Fargo‐Moorhead Diversion Engineering Design <br />OWNER: Metro Flood Diversion Authority <br />The following additions, deletions, or revisions to the Work have been ordered and authorized: <br />OBJECTIVE: <br />Provide an interim budget increase to allow Terracon Consultants, Inc. to continue performing Owner’s Quality <br />Assurance (QA) testing on Work Packages 42A.1/A.3 (4th Street Pump Station) and 42A.2 (2nd Street Pump Station). <br />BACKGROUND: <br />Original budget for Terracon Consultants, Inc. Task Order No. 1 was estimated prior to design completion of these <br />Work Packages. Now that design is complete and construction underway, PMC has a better understanding of the <br />frequency of QA testing to be performed by Terracon Consultants, Inc., which is greater than the original budget <br />can provide for. PMC is currently developing an estimate for a new budget that will extend services for the current <br />known requirements of these Work Packages, and which will be presented to the Diversion Authority in a future <br />amendment. In the meantime, an interim budget increase is necessary so that Terracon Consultants, Inc. can <br />provide uninterrupted material sampling and testing services in support of the Owner’s QA documentation <br />requirements. The requested increase is estimated to provide six (6) additional weeks of service based on average <br />weekly cost to date under the current Task Order. <br />SCOPE: <br />Provide on‐call material sampling and testing services for construction QA requirements including soil compaction <br />and bearing strength; concrete temperature, air content, slump, and compressive strength; reinforcing steel <br />inspection; contamination characterization; product property testing (e.g., rubber waterstops); and other items as <br />requested. <br />DELIVERABLES: <br />Field observation reports, laboratory test results, sampling records, etc. <br />SCHEDULE: <br />Begin up on receipt of AWD. <br />HOW WORK IS PERFORMED: <br />This work will be performed on a time and material basis. <br />COST: <br />Cost incurred under this AWD is not to exceed $30,000. Scope and budget changes for the Work will be included in <br />a future amendment to Terracon Consultants, Inc., Task Order No. 1, Amendment 1. <br />REASON FOR CHANGE(S): <br />Continue materials testing for In‐Town Levees (WP‐42). <br />DR <br />A <br />F <br />T
