IV. ACCOUNTING <br /> <br />Accounts and Records. Grantee shall establish and maintain accurate and complete accounts <br />and financial and other records in relation to the receipt and expenditure of any and all grant <br />funds for the project as set forth in the Grant Application, so that they adequately show the use <br />of the grant funds exclusively for the purposes described in the Grant Application. <br />These project accounts and records shall be retained by the Grantee from the Effective Date of <br />this Agreement and for at least three (3) years following the completion of the grant project <br />period. <br /> <br />Audit. The project accounts and records of the Grantee shall be audited in the same manner as <br />all other accounts and records of the Grantee are audited. The Grantee's project accounts and <br />records (including but not limited to accounts and records relating to the project) may be audited <br />and/or inspected on the Grantee's premises or otherwise by individuals or organizations <br />designated and authorized by the Foundation at any time during the period specified in Section <br />Ill(A) above and within three years thereafter. <br /> <br />REPORTS <br /> <br />Grant Reports and Performance Monitoring. The Grantee shall submit to the Foundation periodic <br />interim reports and supporting information for the expenditure of the grant funds in accordance <br />with the Grant Application, including without limitation: 1) a financial report comparing the <br />amounts actually expended for each item with the budgeted amount approved by the Foundation <br />in connection with the Grant Application, and 2) a final report upon completion of the grant <br />period. The grantee shall complete the on-line reports in accordance with the Covering Kids and <br />Families grant reporting requirements. Such reports shall b e submitted to the Foundation by <br />Grantee in accordance with the reporting schedule listed below: <br /> <br />Evaluation reports will be provided on a quarterly basis in April, July, October and January and <br />will include: <br /> <br />2. <br />3. <br />4. <br />5. <br />6. <br /> <br />Documentation of all outreach/enrollment activities: <br />Documentation of all referrals and outcomes; <br />Community response to simplification efforts; <br />Enrollment assistance program evaluation of training programs; <br />Log of client specific barriers with report on recommendations; and a <br />Budget expenditure report. <br /> <br />Grantee shall also submit to the Foundation any other grant reports requested by the <br />Foundation, within a reasonable time period, containing the requested information in the format <br />designated by the Foundation. Grantee shall also supply the Foundation with such information <br />as may be necessary or desirable to permit the Foundation to review the use made of this grant <br />and its effect on the public status of Grantee. <br /> <br />B. Authorized Agent. For purposes of administration of the reporting and compliance provisions of <br /> this agreement, the Foundation's authorized agent is J. Patrick Traynor, President. <br /> <br />Grantee's Financial Status and Organization. Grantee shall supply the Foundation, upon its <br />request, with any information as may be necessary or desirable to permit the Foundation to <br />review Grantee's financial status, organizational structure and management team. Grantee shall <br />notify the Foundation immediately if there is any substantial negative change in Grantee's <br />financial position and/or management team members or organizational structure. <br /> <br />Do <br /> <br />Conflicts of Interest Disclosure Form. Grantee acknowledges that Grantee must continuously <br />disclose Grantee's potential conflicts of interest with the Foundation so that the Foundation can <br />protect its tax-exempt status by avoiding transactions, arrangements, or uses of the <br />Foundation's funds that might unreasonably benefit the private interests of Grantee, including <br />benefits to Grantee, Grantee's directors, trustees, officers, committee members or key <br /> <br /> <br />
