PURCHASE OF SERVICE AGREEMENT WHEREAS, the County of Cass has determined the services referred to the paragraph below entitled "Scope of Services" should be purchased; and WHEREAS, Share House, Inc., a Private non-profit Agency (Provider), proposes to provide these services; NOW, THEREFORE, the County of Cass and Share House, Inc. enter into the following: AGREEMENT I. TERM OF AGREEMENT: The term of this agreement shall be from the 1st day of September, 2001 through the 31st day of August, 2002. II. SCOPE OF SERVICE: The Provider agrees to provide services set forth in the attached proposals. The Provider will accept Cass County inmates for minimum security placement at the facility located at 4227 9th Avenue South in Fargo, North Dakota. The supervision and programs are provided by the staff at Share House Recovery Center and will include lodging, supervision, drug and alcohol testing, job placement, and counseling as appropriate. Additionally, the Provider will accept referrals from the court or SherifFs Department who may be supervised as Day Reporters with or without electronic monitoring and Home Confinement with electronic monitoring cases as a condition of house arrest. Drug and alcohol tests are included for day reporting and home confinement inmates. Day Reporters will also be provided with counseling availability. III. COMPENSATION: A. Compensation for the minimum security services outlined under terms of this agreement shall be $35.00 per day for each placement. Inmates will pay $15.00 per working day toward this costs. The inmates contribution deducted from the monthly billing for each placement.
