06/12/2014 FEDERAL EMERGENCY MANAGEMENT AGENCY HMGP-EV 01 <br /> 1:03 PM HAZARD MITIGATION GRANT PROGRAM <br /> Environmental Report <br /> Disaster FEMA Amendment App ID State Grantee <br /> Number Project Number Number <br /> 1981 24-R 1 25 ND Statewide <br /> Subgrantee: Cass(County) <br /> FIPS Code: 017-99017 Project Title: Lake Shure Bridge <br /> gteauired Conditions Resultina from Environmental Review <br /> Monitoring Monitoring Completed <br /> Law Entered By Required Date By <br /> NHPA Daniel N.Jones ❑ 00/00/0000 <br /> Conditions Gravel and other fill material must be obtained from a NDDOT,THPO or State Historic Preservation Office(SHPO)approved <br /> Required(4000) source.If the contractor chooses to use a borrow source that has not been NDDOT Certified and/or from an existing <br /> stockpile,the following must occur prior to excavation: <br /> 1.The applicant shall notify FEMA,THPO and/or SHPO(as appropriate)of the proposed action and provide required <br /> archeological surveys to FEMA for review and approval,and acceptance by the THPO and/or the SHPO. <br /> 2.The applicant shall notify the FEMA Regional Environmental Officer if any listed species or designated critical habitat <br /> might be affected or is in the vicinity of the project,and shall not begin work until notified by the FEMA Regional <br /> Environmental Officer that the requirements of the Endangered Species Act have been satisfied. <br /> 3.Applicant is required to coordinate borrow pit locations with the US Fish and Wildlife Service(USFWS)Wetland District <br /> Manager to avoid project impacts to easements or public lands.Project Officerts have notified the Applicant of the <br /> requirement of proof of coordination and documentation. <br /> 4.Applicant is responsible for ensuring that no borrow pit activity occurs in Waters of the United States,wetlands or a river <br /> listed on the Nationwide Rivers Inventory(NRI).Borrow pit activity occurring in a floodplain must not affect pre-existing <br /> hydrological profiles.If activities occur in prime farmland ground disturbance must be temporary,one time use,and no <br /> permanent loss of farmland may occur. <br /> SFW&NH Daniel N.Jones ❑ 00/00/0000 <br /> Conditions 1. The sub-applicant must follow the ND Game and Fish Department regulations outlined in the letter dated 3/17/14 <br /> Required(4000) regarding Aquatic Nuisance Species(ANS). <br /> 2. Disturbed areas should be re-seeded with a native grass mixture. <br /> 3. In order to minimize impacts to the adjacent wetlands,appropriate erosion and sediment control measures must be <br /> installed to control the discharge of pollutants from the construction site. <br /> 4. Any disruption or displacement of the streambed and banks other than the planned alterations must be restored to <br /> pre-project conditions. <br /> 5. If a temporary bypass road is required,it should be designed to accommodate the migration of aquatic species. <br /> SW&S Daniel N.Jones ❑ 00/00/0000 <br /> Conditions 1. Applicant shall comply with proper construction practices and environmental disturbance requirements as outlined in the <br /> Required(4000) NDDOH letter dated 12/19/11. <br /> 2. Care is to be taken during demolition activity near any water of the state to minimize adverse effects on a water body. <br /> This includes minimal disturbance of stream beds and banks to prevent excess siltation,and the replacement and <br /> revegetation of any disturbed area as soon as possible after work has been completed.Caution must also be taken to <br /> prevent spills of oil and grease that may reach the receiving water from equipment maintenance,and/or the handling of fuels <br /> on the site.Guidelines for minimizing degradation to waterways during construction are attached to the letter. <br /> 3. Projects disturbing one or more acres are required to have a permit to discharge storm water runoff until the site is <br /> stabilized by the reestablishment of vegetation or other permanent cover.Further information on the storm water permit may <br /> be obtained from the Department's website or by calling the Division of Water Quality(701-328-5210).Also,cities may <br /> impose additional requirements and/or specific best management practices for construction affecting their storm drainage <br /> system. Check with the local officials to be sure any local storm water management considerations are addressed. <br /> Page 3 of 4 <br />
