City of Moorhead <br /> 2014 ADULT MOSQUITO SURVEILLANCE AGREEMENT <br /> This agreement for adult mosquito surveillance is made between the City of Moorhead whose office is at Po <br /> Box 779,500 Center Ave Moorhead,MN 56561 hereinafter CONTRACT HOLDER,and the Cass County <br /> Commission,whose office is at 211 Ninth Street,Fargo,North Dakota 58108. Mailing address is Box 2806 <br /> Fargo,ND 58108 hereinafter COUNTY. <br /> THE PARTIES STIPULATE AND AGREE AS FOLLOWS: <br /> 1) COUNTY agrees to provide adult mosquito surveillance activities to CONTRACT HOLDER <br /> commensurate with the COUNTY resources available. These activities include:collection of adult <br /> mosquito traps,specimen identification and data reporting. <br /> 2) CONTRACT HOLDER agrees to provide full funding for all labor,supplies,and additional cost <br /> of any and all activities within and adjacent to CONTRACT HOLDER as requested by <br /> CONTRACT HOLDER. <br /> 3) Estimated annual cost for items in paragraph 1 is$5,500.Estimated start and end dates for the <br /> above activities is May 15th to Sept. 15th respectively. <br /> 4) COUNTY will provide monthly statements to CONTRACT HOLDER of labor hours and <br /> materials,and invoice CONTRACT HOLDER monthly.Payments for services are to be made <br /> within 30 days. <br /> 5) The County Vector Director and other fulltime COUNTY staff will provide technical direction, <br /> coordination of activities,public relations,and any other miscellaneous activities as requested. <br /> The Vector Director will also work in cooperation with CONTRACT HOLDER officials to <br /> provide other technical and administrative assistance as necessary. <br /> 6) CONTRACT HOLDER agrees to provide funding for a fixed fee not to exceed$4,000 for <br /> services rendered in paragraph 5 as well as fuel,insurance,and other miscellanous costs <br /> occurred.This will appear as a$1000 administration charge on each monthly statement. <br /> 7) If at any time total charges to CONRACT HOLDER exceeds$9500 the CONTRACT HOLDER <br /> can request a stoppage in service. <br /> To request a stoppage in service,CONTRACT HOLDER must provide COUNTY written notice <br /> 14 days in advance of requested termination to Cass County Vector Control at the following <br /> address: <br /> 1201 Main Ave West <br /> West Fargo,ND 58078 <br />
