Please have the County Commission Chairman sign the top and bottom of page 5 of the agreement, make a copy for your file, and then mail the original back to me. The grant has been assigned a number of JABG(L)-2013-001. Please use this number as a reference on all future correspondence relating to this grant. The necessary forms for reporting and expense reimbursement under the grant can be found at: http://www.ndaco.org/jj_reporting. Financial and program progress reports are 15 days following the end of each quarterly period(i.e. 4/15/14; 7/15/14; 10/15/14; 1 /15/15). A reimbursement of grant funds can be requested at any point during the grant period using the request for funds form. If you have any questions, please do not hesitate to contact me.
