Box 488 <br /> Fargo, ND 58107-0488 <br /> Investigator Burris, <br /> RE: Automated Pawn System TM Service and Software License Agreement <br /> Thank you for requesting information about joining APS as a Query Only agency, we are pleased <br /> to provide you with the following documents for your consideration: <br /> • Automated Pawn System TM (APS)Service and Software License Agreements—(Print <br /> have signed and mail back two originals). <br /> Please return both of the signed originals when you have completed your approval <br /> process. After they are signed and accepted by Minneapolis, we will return one fully <br /> executed original agreement to you for your records. Mail originals to: <br /> Minneapolis Police, Attn: Mary Sullivan, 350 South gh St, Rm 127, Mpls, MN 55415. <br /> • New User Information form (Forms are pages 2&3 of this document): <br /> Complete both pages scan and email(or fax) them, along with a signed Signature Page <br /> from the License Agreement, to Mary Sullivan, APS System Administrator, (612) 673-2619 <br /> or mars.sullivan(&,minneapolismn.gov <br /> We will begin connecting you as soon as we receive the above items and we will schedule user <br /> training at that time, most likely using "Go-to-Meeting" as a platform. Your IT Department may <br /> install APS on as many pc's in your Department as you desire. You can set up as many users of <br /> APS as you need with their own User ID and password, there is no charge for additional users <br /> and they can do unlimited queries. <br /> The only cost to your Department is an annual User Access fee of$204 (for an unlimited number <br /> of users to do unlimited number of queries)which will be prorated for the remainder of 2012 <br /> (17.00 per month for 3 mo's) and as you requested, we will include the $204 charge for 2014 and <br /> invoice you in November for a total amount of$255.00.
